St. Anthony Health Care Center provides skilled nursing care 24 hours a day for both acute and subacutc conditions. We accept Medicare and Medicaid certified patients as well as private Insurance for either short or long term stays.

Our individual approach to nursing involves a variety of health care specialties coordinating individualized care based on personal assessments and team rounds. Determining the best care for your loved one is our priority. Personalized care plans are implemented for the continued improvement and maintenance of residents' health status.

St. Anthony coordinates resident psychosocial care through our in house licensed social worker. Our social worker assists with the psychological and emotional needs of both the family and residents. A team that consists of our social worker, residents, hospice nurses and families create hospice care and discharge planning when needed. St. Anthony provides home health service evaluations and coordinates home care programs to maximize and maintain the functional ability of the residents, as they become well enough to leave our care.
